The Bella Luza Skin Consultant is committed to maintaining privacy and confidentiality at all times and requires that any information regarding individual patients, including staff members who may be patients, will not be disclosed in any form (verbally, in writing, or electronically, inside or outside our practice) except for strictly authorised use within the patient care context or as required by law.
For the purposes of this policy, no distinction has been made between the handling of personal information and sensitive information (including health information), therefore all information will be referred to as “personal information” throughout this Policy.

- Sensitive information about a patient such as but not limited to; past medical history, medications, allergies, social history, family history, cultural background
Use and Disclosure Information
The primary purpose in collecting and holding personal information is to provide comprehensive skin consultation and product advice.
Team members working for The Bella Luza Skin Consultants receive training on the importance of keeping medical and other personal information confidential and are bound by internal information security policies and confidentiality obligations.
The Bella Luza Skin Consultants also conveys this information electronically in encrypted form to the appropriate compounding pharmacy to create customised skincare products on prescription.
The pharmacy making your Bella Luza Skincare products are also required by law to collect and disclose certain information when they process prescriptions including compounded prescriptions.
The information required by the pharmacy may be store in electronic databases with access limited to persons issued with secure passwords. Paper records containing sensitive information are stored separately from other pharmacy records or in hard copy form both at our premised and with assistance from our service providers.
